You can apply for a 'new connection, or modify an existing connection', with us by completing one of the application forms below and sending it to our Records Team by email or post.
Once we’ve received your application, we will contact you within two days to discuss your scheme.

When you have completed your application, please email or post it, along with its supporting documentation, to your regional office listed below.
Lines are open Monday to Friday, 9:00am to 5:00pm.
